老年难治性巨大结石胆管支架联合药物治疗1例并文献复习
A Case of Elderly Refractory Giant Stone Bile Duct Stent Combined with Pharmacological Treatment and Review of Literature
摘要: 随着现如今介入及内镜技术的发展,内镜下逆行胆胰管造影术(ERCP)治疗胆总管结石已经成为一种安全且创伤小的手术方式,而对于内镜下取石困难的胆总管结石患者目前多行内镜下胆管支架置入引流术(ERBD)后择期内镜下取石,此种方法存在更好的有效性与安全性。ERBD术后的难治性胆总管结石存在支架“溶石”现象,即胆道支架通过物理摩擦等方式缩小胆总管结石直径的现象,但相关临床数据较少。本文报道1例难治性胆总管巨大结石接受ERBD联合熊去氧胆酸治疗的案例。
Abstract: With the development of interventional and endoscopic techniques, endoscopic retrograde cholangiopancreatography (ERCP) has become a safe and less invasive procedure for the treatment of common bile duct stones, and for patients with difficult choledocholithiasis, endoscopic retrograde biliary drainage (ERBD) followed by elective endoscopic stone extraction is now more effective and safe. The phenomenon of stent “litholysis”, in which biliary stents reduce the diameter of common bile duct stones by physical friction, exists in refractory common bile duct stones, but there are few relevant clinical data. In this paper, we report a case of refractory common bile duct stones treated with ERBD combined with ursodeoxycholic acid.
