摘要: 食管胃静脉曲张是肝硬化的常见并发症之一,与出血风险紧密相关,可导致不良预后及转归,评估其存在及严重程度对指导临床治疗及改善预后具有重要意义。胃镜作为静脉曲张诊断的金标准,因其侵入性、耐受性差等因素,使得部分患者依从性较差,不能及时随访及评估。近年来,预测食管胃静脉曲张发生的无创指标被提出,其中研究最广泛的包括血清学指标、多普勒超声、CT、磁共振成像、弹性成像技术等,除此之外,新兴的组学、人工智能等技术也应用于对静脉曲张进行无创预测,并展现出了良好的诊断性能。我们对现有的无创评估方法进行了整理总结,分析其优势及存在的不足,为肝硬化食管胃静脉曲张的非侵入性诊断提供了新思路。
Abstract: Esophagogastric varices are one of the common complications in liver cirrhosis, which are closely related to the risk of bleeding and can lead to poor prognosis and outcome. Assessing their presence and severity is of great significance to guide clinical treatment and improve prognosis. Gastroscopy, the gold standard for variceal diagnosis, is invasive and poorly tolerated, making some patients poorly compliant and unable to be followed up and evaluated in a timely manner. In recent years, noninvasive indicators for predicting the occurrence of esophagogastric varices have been proposed, among which the most widely studied include serological indicators, Doppler ultrasound, computed tomography, magnetic resonance imaging, elastography, etc. In addition, emerging omics and artifi-cial intelligence have also been applied to noninvasive prediction for esophagogastric varices, and have shown good diagnostic performance. We summarized the existing noninvasive evaluation methods, analyzed their advantages and shortcomings, and provided new ideas for the noninvasive diagnosis of esophagogastric varices in liver cirrhosis.
