双源螺旋CT测量腹部脐层面VAT面积与冠心病的相关性研究

摘要: 目的:探讨双源螺旋CT (dual source computed tomography, DSCT)测量腹部脐层面内脏脂肪组织(visceral adipose tissue, VAT)面积与冠心病之间的相关性。方法:选取2019年10月~2020年6月期间在我院心内科怀疑冠心病行冠状动脉造影的124例患者作为研究对象,其中男62例、女62例,年龄34~85岁,平均年龄(61.31 ± 0.89)岁。根据冠状动脉造影结果是否确诊冠心病分为非冠心病组(n = 60)与冠心病组(n = 64)。比较两组的临床特征、实验室指标以及腹部脐层面VAT面积值。结果:非冠心病组年龄、肌酐低于冠心病组,差异具有统计学意义(P值均<0.05),HDL、LDL高于冠心病组,差异具有统计学意义(P值均<0.05),性别、吸烟与否、高血压与否、糖尿病与否、尿酸、总胆固醇、甘油三酯差异无统计学意义(P值均>0.05)。冠心病组腹部脐层面VAT面积显著高于非冠心病组(P < 0.05)。结论:除常规的临床及实验室指标外,腹部脐层面VAT面积亦可作为冠心病危险评估的有效指标。
Abstract: Objective: To explore the relationship between the area of visceral adipose tissue (VAT) measured by dual-source spiral CT (DSCT) and coronary heart disease (CHD). Methods: 124 patients with sus-pected coronary heart disease underwent coronary angiography in the department of cardiology of our hospital from October 2019 to June 2020, including 62 males and 62 females, aged from 34 to 85 years, with an average age of (61.31 ± 0.89) years. According to the results of coronary angi-ography, patients with coronary heart disease were divided into two groups: non-coronary heart disease group (n = 60) and coronary heart disease group (n = 64). The clinical features, laboratory indexes and VAT area of abdominal umbilical plane were compared between the two groups. Re-sults: The age and creatinine in the non-coronary heart disease group were significantly lower than those in the coronary heart disease group (P < 0.05), and the HDL and LDL in the non- coronary heart disease group were significantly higher than those in the coronary heart disease group (P < 0.05). There was no significant difference in sex, smoking, hypertension, diabetes, uric acid, total cholesterol and triglyceride (P > 0.05). The area of VAT on the umbilical plane of abdomen in CHD group was significantly higher than that in non-CHD group (P < 0.05). Conclusion: In addition to rou-tine clinical and laboratory indexes, the area of VAT on abdominal umbilical plane can also be used as an effective index for risk assessment of coronary heart disease.
