芪柏愈疡油联合负压封闭引流术治疗糖尿病足的临床观察

摘要: 目的:观察芪柏愈疡油联合负压封闭引流术治疗糖尿病足(diabetic foot, DF)的疗效和安全性。方法:以2021年10月~2022年09月符合纳入标准的84例糖尿病足溃疡患者为观察对象,按照随机数字表法将患者分为两组,对照组和观察组各42例。两组患者均予基础治疗和常规清创换药,在此基础上,对照组予康复新液联合负压封闭引流术(vacuum sealing drainage, VSD)治疗,观察组予芪柏愈疡油联合负压封闭引流术治疗,连续治疗28 d。比较两组患者治疗前后的临床疗效、创口愈合速度、创口愈合率、白细胞计数、超敏C反应蛋白值、创口量化评分(渗出液、肉芽组织及疼痛)及不良反应。结果:两组临床疗效相比较,观察组整体临床疗效优于对照组,差异有统计学意义(P < 0.05)。观察组的创口愈合速度、创口愈合率比对照组明显提高,差异有统计学意义(P < 0.05)。治疗后的白细胞计数、超敏C反应蛋白值、创口量化评分(渗出液、肉芽组织及疼痛)较治疗前明显降低(P < 0.05),且观察组比对照组降低更明显,差异有统计学意义(P < 0.05)。此外,两组患者不良反应发生率的差异无统计学意义(P > 0.05)。结论:芪柏愈疡油联合负压封闭引流术治疗糖尿病足溃疡能有效减轻炎症感染,保持创口洁净,改善局部血液供应,促进肉芽组织形成,从而明显加快创口愈合,且安全性具有保障,此治疗方案适宜在临床中推广应用。
Abstract: Objective: To observe the efficacy and safety of Qibai Yuyang oil combined with vacuum sealing drainage in the treatment of diabetic foot (DF). Methods: From October 2021 to September 2022, 84 patients with diabetic foot ulcer who met the inclusion criteria were observed. The patients were divided into two groups according to the random number table, 42 patients in the control group and 42 patients in the observation group. Both groups were treated with basic treatment and con-ventional debridement and dressing change. On this basis, the control group was treated with Kangfuxin solution combined with vacuum sealing drainage (VSD), and the observation group was treated with Qibai Yuyang oil combined with vacuum sealing drainage for 28 consecutive days. The clinical efficacy, wound healing speed, wound healing rate, white blood cell count, hypersensitive C-reactive protein value, wound quantitative score (exudate, granulation tissue and pain) and ad-verse reactions of the two groups were compared before and after treatment. Results: Compared with the control group, the overall clinical efficacy of the observation group was better than that of the control group, with a statistically significant difference (P < 0.05). The speed and rate of wound healing in the observation group were significantly higher than those in the control group (P < 0.05). The white blood cell count, hypersensitivity C-reactive protein value and wound quantitative score (exudate, granulation tissue and pain) after treatment were significantly lower than those before treatment (P < 0.05), and the decrease in the observation group was more significant than that in the control group, with a statistically significant difference (P < 0.05). In addition, there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion: Qibai Yuyang Oil combined with negative pressure sealing drainage can effectively re-duce inflammation and infection, keep the wound clean, improve local blood supply, promote gran-ulation tissue formation, thus significantly accelerate wound healing, and ensure safety. This treat-ment scheme is suitable for clinical application.
