胰十二指肠切除术后迟发性出血的研究现况

Current Status of Research on Delayed Bleeding after Pancreaticoduodenectomy

摘要: 目的：总结胰十二指肠切除术后迟发性出血的研究进展，旨在探讨胰十二指肠切除术后出血的危险因素，以及为探寻预测胰十二指肠切除术后迟发性出血提供思路。方法：检索胰十二指肠切除术后出血的相关文献，并作一综述。结果：目前国内外均有使用C反应蛋白、降钙素原、白细胞等炎症指标早期预测胰瘘、腹腔感染、假性动脉瘤的研究，但使用血液标志物预测术胰腺后迟发性出血的研究较少。结论：胰十二指肠切除术后迟发性出血是一个持续且复杂的过程，但通过研究可以发现，迟发性出血的发生发展均离不开炎症反应，寻找能够预测胰十二指肠切除术后迟发性出血的敏感性及特异性较高的炎症指标具有一定的临床意义及研究价值。

Abstract: Objective: To summarize the research progress of delayed bleeding after pancreaticoduodenectomy, we aim to investigate the risk factors of bleeding after pancreaticoduodenectomy, as well as to pro-vide ideas to explore the prediction of delayed bleeding after pancreaticoduodenectomy. Methods: The literature related to post-pancreaticoduodenectomy hemorrhage was searched and a review was presented. Results: Early prediction of pancreatic fistula, abdominal infection, and pseudoan-eurysm using inflammatory indicators such as C-reactive protein, calcitoninogen, and leukocytes has been studied both nationally and internationally, but fewer studies have used blood markers to predict late postoperative pancreatic hemorrhage. Conclusion: Delayed bleeding after pancreati-coduodenectomy is a continuous and complex process, but it can be found that the development of delayed bleeding is inseparable from the inflammatory response, so it is of clinical significance and research value to find inflammatory indicators with high sensitivity and specificity that can predict delayed bleeding after pancreaticoduodenectomy.
