外周血复合炎症指标与慢性鼻窦炎伴鼻息肉的相关性分析

Correlation Analysis of Peripheral Blood Complex Inflammation Index and Chronic Sinusitis with Nasal Polyps

摘要: 目的：分析慢性鼻窦炎伴鼻息肉患者与正常人群外周血复合炎症指标的差异，评估其对慢性鼻窦炎伴鼻息肉患者预诊断价值，及其与鼻内镜、鼻窦CT评分的相关性，寻找一种方便、可行的慢性鼻窦炎伴鼻息肉预诊断及病情评估方法。方法：根据是否患有鼻息肉进行分组，收集两组血常规等临床资料，并计算NLR、ELR、ENR等复合炎症指标，运用Mann-Whitney U检验、Student’s t检验和Pearson相关性分析等统计学方法，分析比较两组别间的差异及鼻息肉患者相关生物指标间的相关性，并使用ROC曲线评估各指标对鼻息肉患者的预诊断价值。结果：鼻息肉患者复合炎症指标EOS%、NLR、ELR、PLR、ENR、SII、SIRI高于正常人群，LMR低于正常人群，差异均存在统计学意义(P < 0.05)，其中NLR (AUC = 0.717)、SII (AUC = 0.758)和SIRI (AUC = 0.730)的预诊断效果较好；EOS%、ELR、ENR与鼻息肉患者CT评分、主观症状评分呈正相关。结论：慢性鼻窦炎伴鼻息肉患者外周血EOS%、NLR、ELR等复合炎症指标与正常人群存在显著差异；NLR、SII和SIRI预诊断效果好，可为慢性鼻窦炎伴鼻息肉患者的门诊筛查提供参考；EOS%、ELR、ENR可以初步判断患者的鼻息肉严重程度，为患者用药提供参考，减少抗生素及激素的滥用。

Abstract: Objective: To analyze the difference of peripheral blood complex inflammation index between pa-tients with chronic sinusitis and nasal polyps and normal people, evaluate its diagnostic value for patients with chronic sinusitis and nasal polyps, and its correlation with nasal endoscopy and sinus CT score, and find a convenient and feasible method for the diagnosis and evaluation of chronic si-nusitis and nasal polyps. Methods: The patients were divided into groups according to whether they had nasal polyps, collected clinical data such as blood routine test, and calculated NLR, ELR, ENR and other composite inflammatory indicators. The differences between the two groups and the cor-relation between biological indicators related to nasal polyps were analyzed and compared using statistical methods such as Mann-Whitney U test, Student’s t test, Pearson correlation analysis, and the ROC curve was used to evaluate the predictive value of each indicator for patients with nasal polyps. Results: EOS%, NLR, ELR, PLR, ENR, SII, SIRI of nasal polyp patients were higher than those of the normal population, and LMR was lower than those of the normal population. The difference was statistically significant (P < 0.05); Among these metrics, NLR (AUC = 0.717), SII (AUC = 0.758) and SIRI (AUC = 0.730) have good predictive efficacy. EOS%, ELR and ENR were positively correlat-ed with CT score and subjective symptom score of patients with nasal polyps. Conclusion: The EOS%, NLR, ELR and other complex inflammatory indexes in the peripheral blood of patients with chronic sinusitis and nasal polyps are significantly different from those of normal people; NLR, SII and SIRI have good pre-diagnosis effect, which can provide reference for outpatient screening of patients with chronic sinusitis and nasal polyps; EOS%, ELR and ENR can preliminarily determine the sever-ity of nasal polyps in patients, provide reference for patients in medication, and reduce the abuse of antibiotics and hormones.
