三维斑点追踪技术评价不同左室构型尿毒症患者左心房功能
Evaluation of Left Atrial Function in Uremic Patients with Different Left Ventricular Configurations by Three-Dimensional Speckle Tracking Imaging
摘要: 目的:探讨三维斑点追踪技术(3D-STI)评价不同左心室构型尿毒症患者左心房功能的价值。方法:将98例尿毒症患者根据不同左室构型分为4组:正常构型31例(NG组),向心性重构22例(CR组),向心性肥厚24例(CH组),离心性肥厚21例(EH组),另选30例健康志愿者作为对照组;应用三维斑点追踪技术,分析并对比各组左房整体圆周应变(GCS)、整体纵向应变(GLS)及整体径向应变(GRS)的差异。结果:与正常对照组比较,各病例组GLS均减低,CR组、CH组、EH组GRS、GCS均减低(P < 0.05);与NG组比较,CR组、CH组、EH组GCS减低,CH组、EH组GLS、GRS均减低(P < 0.05);与CR组比较,CH组、EH组GLS、GRS、GCS均减低(P < 0.05);与CH组比较,EH组GLS、GRS、GCS均减低(P < 0.05)。结论:三维斑点追踪技术评价不同左心室构型尿毒症患者的左心房功能有较好的临床价值。
Abstract: Objective: To explore the value of three-dimensional speckle tracking imaging (3D-STI) in evaluat-ing left atrial function in uremic patients with different left ventricular configurations. Methods: 98 patients with uremia were divided into 4 groups according to different left ventricular configuration: normal configuration 31 cases (NG group), concentric remodeling 22 cases (CR group), concentric hypertrophy 24 cases (CH group), eccentric hypertrophy 21 cases (EH group), and 30 healthy vol-unteers were selected as control group. Three-dimensional speckle tracking technique was used to analyze and compare the differences of global circumferential strain (GCS), global longitudinal strain (GLS) and global radial strain (GRS) of left atrium in each group. Results: Compared with normal control group, GLS, GRS and GCS in CR group, CH group and EH group decreased (P < 0.05), GCS decreased in CR group, CH group and EH group, GLS and GRS decreased in CH group and EH group (P < 0.05), GLS, GRS and GCS in CH group and EH group decreased compared with CR group (P < 0.05), GLS, GRS and GCS decreased in EH group compared with CH group (P < 0.05). Conclusion: Three-dimensional speckle tracking imaging is of good clinical value in evaluating left atrial func-tion in uremic patients with different left ventricular configuration.
