阴道镜宫颈活检在诊断高级别鳞状上皮内病变的应用价值及其影响因素
Application Value and Influencing Factors of Colposcopic Cervical Biopsy in Diagnosis of High-Grade Squamous Intraepithelial Lesions
摘要: 宫颈癌作为女性最常见的恶性肿瘤之一。宫颈癌变过程是极其复杂的过程,其中宫颈鳞状上皮内病变(SIL)是关键一步,它由人乳头瘤病毒(HPV)感染导致的一种病理现象。根据癌变可能性将其分为子宫颈低级别鳞状上皮内病变(LSIL)和高级别鳞状上皮内病变(HSIL)。其中HSIL的癌变率极高。准确筛查出HSIL并为患者制定个体化诊疗方案对预防宫颈癌的发生和发展具有重要意义。阴道镜下活检作为宫颈病变诊断的“金标准”,但临床上阴道镜下活检受到多种因素的影响导致其准确性有待于提高。本文就阴道镜宫颈活检在诊断HSIL的应用价值及其影响因素作一综述。
Abstract: Cervical cancer is one of the most common malignant tumors in women. Cervical cancer is a compli-cated process, and cervical squamous intraepithelial lesion (SIL), a pathological phenomenon caused by human papillomavirus (HPV) infection, is a key step. They were classified into low-grade intraepithelial lesions (LSIL) and high-grade intraepithelial lesions (HSIL) according to their car-cinogenesis potential. The cancer rate of HSIL was very high. It is of great significance to accurately detect HSIL and develop individualized diagnosis and treatment plan for patients to prevent the occurrence and development of cervical cancer. Colposcopic biopsy is the “gold standard” for the diagnosis of cervical lesions, but its accuracy needs to be improved due to the influence of many factors. This article reviews the application value and influencing factors of colposcopic cervical bi-opsy in the diagnosis of HSIL.
