新冠病毒感染后头痛的临床调查研究

摘要: 目的:分析COVID-19大流行后头痛患者的临床特征,为临床诊疗提供科学依据。方法:收集自2023.01.20~2023.02.20在山东第一医科大学第二附属医院神经内科门诊就诊的头痛患者258例,调查其一般情况、新冠感染情况、头痛情况等,统计分析以明确COVID后头痛的临床特征。结果:本研究结果显示COVID后头痛患者以中青年女性为主,既往无头痛病史者多见。在既往头痛病史患者中,偏头痛所占比例最高。COVID后头痛性质以在既往头痛基础上症状加重者多见,头痛程度多为轻中度,阵发性所占比例较高,头痛部位以双侧颞部最常见,疼痛性质以胀痛为主,伴随症状多为全身乏力、头晕、后颈部疼痛或僵硬感等。新冠病毒感染后1月有14.34%病例遗留头痛。结论:目前已知晓COVID后头痛在疾病急性期的临床特征,但仍需要对其存在的后遗症进行持续管理和研究。
Abstract: Objective: To analyze the clinical characteristics of headache patients after COVID-19 pandemic and provide scientific basis for clinical diagnosis and treatment. Methods: 258 headache patients were collected from the Department of Neurology of the Second Affiliated Hospital of Shandong First Medical University from January 20, 2023 to February 20, 2023. Their general conditions, COVID-19 infection and headache were investigated, and the clinical characteristics of headache after COVID were statistically analyzed. Results: The results of this study showed that the majority of headache patients after COVID were middle-aged and young women, and those without a previous history of headache were more common. Among patients with a past history of headache, migraine accounts for the highest proportion. The nature of headache after COVID is more common in patients with worsening symptoms based on previous headaches. The degree of headache is mostly mild to mod-erate, with a high proportion of paroxysmal symptoms. The most common headache sites are bilat-eral temporal regions, with pain mainly characterized by bloating and pain. The accompanying symptoms are mostly general fatigue, dizziness, pain in the back neck, or stiffness. One month after COVID-19 infection, 14.34% of cases left headache. Conclusion: The clinical characteristics of post COVID headache in the acute phase of the disease are currently known, but it is still necessary to continue management and research on its existing sequelae.
