喉癌前病变与HPV感染的相关性研究

A Study on the Correlation between Laryngeal Precancerous Lesions and HPV Infection

摘要: 目的：探讨喉癌前病变与HPV感染之间的相关性。方法：对本院30例喉癌前病变组织标本(喉癌前病变组)并以30例声带息肉患者标本为对照(声带息肉组)进行37种HPV亚型检测、HPV DNA检测、HPV mRNA提取。结果：喉癌前病变组HPV阳性率为16.66%，声带息肉组HPV阳性率为3.33%，喉癌前病变组HPV感染阳性的喉癌标本共5例，其中共检测出HPV毒株6株，其中67单一型感染1例，16型单一感染3例，16、45混合型感染1例，喉癌HPV阳性率在不同年龄、性别、病程患者对比中无差异，且喉癌前病变组患者组织HPV DNA检测阳性5例，HPV E7 mRNA拷贝数为<10³ 2例，>10³且<10⁴ 3例。结论：喉癌前病变患者的发病、恶化与HPV感染存在一定的关联，HPV病毒在诱发喉癌前病变演变为喉癌的过程中有一定的作用，但喉癌前病变后患者感染HPV与其性别、年龄、病程并无明显关联。

Abstract: Objective: To explore the correlation between laryngeal precancerous lesions and HPV infection. Method: 37 HPV subtypes, HPV DNA, and HPV mRNA extraction were performed on 30 specimens of laryngeal precancerous lesions in our hospital (laryngeal precancerous lesion group) and 30 speci-mens of vocal cord polyps patients as controls (vocal cord polyp group). Result: The HPV positivity rate in the precancerous lesion group of the larynx was 16.66%, and the HPV positivity rate in the vocal cord polyp group was 3.33%. There were a total of 5 laryngeal cancer specimens with HPV in-fection positivity in the precancerous lesion group of the larynx, of which 6 HPV strains were de-tected, among them, 1 case of 67 monotypic infections, 3 cases of 16 single type infection, and 1 case of 16, 45 mixed type infection. The HPV positivity rate in laryngeal cancer did not differ among pa-tients of different ages, genders, and disease courses, and 5 cases of tissue HPV DNA test positivity in the precancerous lesion group of the larynx. HPV E7 mRNA copy numbers were <10³ in 2 cas-es, >10³ and <10⁴ in 3 cases. Conclusion: There is a certain correlation between the onset and dete-rioration of laryngeal precancerous lesions and HPV infection. HPV virus plays a certain role in in-ducing the progression of laryngeal precancerous lesions to laryngeal cancer, but there is no signif-icant correlation between HPV infection in patients with laryngeal precancerous lesions and their gender, age, and course of disease.
