刘晓鹰教授运用经方治疗小儿杂病验案两则

Professor Liu Xiaoying’s Two Cases of Treating Miscellaneous Diseases in Children with Classic Prescriptions

摘要: 经方是指出自《伤寒论》《金匮要略》的药方，因组方严谨，药少力专而备受历代医家推崇。儿童因其特殊的生理病理特点，在临床上常见疑难杂病，传统治疗疗效不佳时，通过辨证论治，抓住病机，采用经方对证治疗，往往可收获良效。吴茱萸汤为温中补虚、降逆止呕之名方，可主治肝胃虚寒、浊阴上逆证；桂枝汤乃群方之首，外证得之，解肌和营卫；内证得之，化气调阴阳。主治外感风寒表虚证。通过整理刘晓鹰教授应用以上两方分别治疗儿童功能性腹痛、小儿胸痛的医案，来阐述其对吴茱萸汤、桂枝汤的临证辨证思路。刘教授在临床之中对经方的运用灵活多变，从不拘泥于其主治，在具体运用之中，始终结合小儿特有的生理病理特点，顾护小儿脾胃，抓主证用经方，或仿经方之意灵活地运用经方，方证对应，疗效显著。

Abstract: Jing Fang is a medicinal formula from the Treatise on Typhoid Fever and the Essentials of the Golden Chamber, which has been highly respected by medical practitioners throughout the ages for its rigorous formulation and less specialized medication. Because of its special physiopathological characteristics, children are often difficult and miscellaneous diseases in the clinic, and when the traditional treatment is not effective, through the diagnosis and treatment, seize the disease mechanism, and use the Jing Fang for the treatment of the disease, which can often reap good results. Wu Zhuyu Tang is a famous formula for warming the middle and replenishing the deficiency, lowering the rebelliousness and stopping vomiting, which can be used to treat the deficiency and coldness of the liver and stomach and the rebelliousness of the turbid yin; Gui Zhi Tang is the first of the group of prescriptions, which can relieve the muscle and the Ying and Wei in the external cases; and in the internal cases, which can adjust the yin and yang in the internal cases. It is the main treatment for external wind-cold and surface deficiency. By organizing the cases of Professor Liu Xiaoying’s application of the above two formulas in treating children’s functional abdominal pain and pediatric chest pain, respectively, to illustrate his thoughts on the clinical diagnosis of Wu Zhuyu Tang and Gui Zhi Tang. Prof Liu’s flexible use of sutra prescriptions in clinical practice is never restricted to their main treatment, and in specific application, he always combines the unique physiological and pathological characteristics of children, takes care of the children’s spleen and stomach, and uses Jing Fang to grasp the main evidence or uses them flexibly in imitation of Jing Fang, which corresponds to the evidence and produces remarkable therapeutic effects.
