反应性浆细胞增多症合并梅毒及EBV感染1例并文献复习

Reactive Plasmacytosis with Syphilis and EBV Infection: A Case Report and Literature Review

摘要: 目的：报道1例反应性浆细胞增多症(reactive plasmacytosis, RP)合并梅毒(syphilis)及EB病毒(Epstein-Barr virus, EBV)感染病例并探讨其临床表现、病理学特征、治疗及预后。方法：回顾性分析青岛大学附属医院2023年10月收治的1例RP合并感染患者的临床资料，并进行文献复习。结果：患者男性，48岁，首发症状为发热伴贫血，当地医院查骨髓穿刺不能排除是多发性骨髓瘤，入院后完善相关辅助检查，结果尚不满足单克隆浆细胞疾病诊断标准，且合并梅毒及EBV感染，反应性浆细胞增多可能性大。结论：RP不是一种独立的疾病，其临床表现、治疗方式及预后均取决于原发病，需与多发性骨髓瘤(multiple myeloma, MM)等疾病相鉴别。

Abstract: Objective: To report a case of reactive plasmacytosis with syphilis and Epstein-Barr virus infection and to explore its clinical features, pathological characteristics, treatment, and prognosis. Methods: The clinical data of the patient with RP was collected and organized from the Affiliated Hospital of Qingdao University in October 2023. The clinical features, histopathological and immunohisto-chemical characteristics, treatment and prognosis were retrospectively analyzed, with review of re-lated literatures. Results: The patient was a 48-year-old male with fever and anemia as the first symptom, and multiple myeloma could not be excluded by bone marrow aspiration in the local hos-pital. After admission, relevant auxiliary examinations were completed, but the results did not meet the diagnostic criteria of monoclonal plasma cell disease. The patient has a high possibility of reactive plasma cell proliferation due to the combination of syphilis and EBV infection. Conclusion: RP is not an independent disease whose clinical manifestations, treatment methods, and prognosis depend on the primary disease. And it needs to be differentiated from diseases such as multiple myeloma (MM).
