多囊卵巢综合征内分泌改变特征及其对妊娠的影响

摘要: 多囊卵巢综合征(PCOS)是一类内分泌生殖疾病,在临床上多以少/无排卵、临床或生物学高雄激素血症和多囊卵巢为主要特征,其发病机制仍是个谜团。PCOS中多系统功能障碍可概括为内分泌功能障碍、生殖功能障碍、代谢功能障碍和生化功能障碍。而PCOS患者内分泌紊乱对生育的影响越来越受到生殖领域专家及学者的关注。作为PCOS最标志性的内分泌特点,高水平LH/FSH比值可引起排卵障碍,并可能影响颗粒细胞功能以及卵子细胞质量,从而导致患者不孕;然而也有研究者指出其对卵母细胞及胚胎质量无负面影响。了解PCOS患者内分泌异常对生育影响的相关机制,可有利于在日常临床工作中提高PCOS不孕症患者的妊娠率,使PCOS患者获得更优的妊娠结局。
Abstract: Polycystic ovary syndrome (PCOS) is a type of endocrine and reproductive disease characterized by anovulation, clinical or biological hyperandrogenism, and polycystic ovary disease. Its pathogenesis remains a mystery. Multi system dysfunction in PCOS can be summarized as endocrine dysfunction, reproductive dysfunction, metabolic dysfunction, and biochemical dysfunction. The impact of endo-crine disorders on fertility in PCOS patients is receiving increasing attention from reproductive ex-perts and scholars. As the most iconic endocrine characteristic of PCOS, high LH/FSH ratios can cause ovulation disorders and may affect granulosa cell function and egg cell quality, leading to in-fertility in patients; However, some researchers have also pointed out that it has no negative impact on the quality of oocytes and embryos. Understanding the mechanisms underlying the impact of endocrine abnormalities on fertility in patients with PCOS can help improve the pregnancy rate and achieve better pregnancy outcomes in daily clinical practice.
