Percheron动脉闭塞致双侧丘脑及中脑梗死伴癫痫发作1例
Bilateral Thalamic and Mesencephalic Infarction with Epileptic Seizure Caused by Occlusion of the Artery of Percheron: A Case Report
摘要: Percheron动脉(AOP)是一种特殊的血管解剖变异，该血管闭塞通常导致双侧丘脑梗死，伴或不伴中脑累及。AOP梗死较为罕见且临床表现复杂多样，典型三联征为意识障碍、垂直凝视麻痹和记忆障碍，当合并癫痫发作时往往预后较差。现报道1例以意识障碍为首发症状，伴随癫痫发作的AOP梗死患者，探讨其临床表现、诊治经过及预后，并学习国内外相关文献，以期提高对该病的认识，改善其临床症状和预后。
Abstract: The Artery of Percheron (AOP) is a specific vascular anatomic variation that usually results in bilat-eral thalamic infarction, with or without midbrain involvement. AOP infarction is rare and its clini-cal manifestations are complex and varied. The typical triad of AOP infarction is consciousness dis-turbance, vertical gaze paralysis and memory disturbance, and the prognosis is often poor when combined with epilepsy. This paper reports a case of AOP infarction with disturbance of conscious-ness as the first symptom accompanied by epilepsy. The clinical manifestations, treatment and prognosis of AOP infarction are discussed, and relevant literatures at home and abroad are studied in order to improve the understanding of this disease and improve its clinical symptoms and prog-nosis.
