布拉氏酵母菌对小儿AAD预防及对免疫的影响
Effects of Saccharomyces boulardii on Prevention and Immunity of AAD in Children
摘要: 目的:研究应用布拉氏酵母菌散剂预防小儿肺炎抗生素相关性腹泻(AAD)的效果,及其对机体免疫方面的影响。方法:选择我科使用抗生素治疗的75例肺炎患儿,依据不同的治疗方式分为观察组和对照组,观察组32例,对照组43例。对照组使用抗生素、常规对症治疗,观察组则在此基础上预防性应用布拉氏酵母菌散剂。两组患儿出现腹泻后均予以蒙脱石散、补液等对症治疗,比较两组患儿AAD发生情况、腹泻持续时间,及治疗前后的T淋巴细胞亚群、免疫球蛋白水平。结果:观察组患儿AAD的发生率显著低于对照组,腹泻持续时间显著短于对照组(P < 0.05)。治疗后观察组血CD4+、CD4+/CD8+、IgA、IgM和IgG高于对照组,CD8+低于对照组,差异有统计学意义(P < 0.05),治疗前两组血以上指标水平比较差异未见统计学意义(P > 0.05)。结论:应用抗生素治疗的肺炎患儿早期服用布拉氏酵母菌,能够有效降低患儿AAD发生风险,缩短患儿腹泻持续时间,改善机体免疫功能。
Abstract:
Objective: To study the effect of Saccharomyces boulardii powder on the prevention of pediatric pneumonia antibiotic-associated diarrhea (AAD) and its effect on immunity. Methods: 75 children with pneumonia treated with antibiotics were divided into observation group (32 cases) and control group (43 cases) according to different treatment methods. The control group was treated with an-tibiotics and conventional symptomatic treatment, and the observation group was treated with prophylaxis of Saccharomyces boulardii powder on this basis. Patients in both groups were given symptomatic treatment such as montmorillonite powder and rehydration after diarrhea. The oc-currence of AAD, the duration of diarrhea, and the levels of T lymphocyte subsets and immuno-globulin before and after treatment were compared between the two groups. Results: The incidence of AAD in observation group was significantly lower than that in control group, and the duration of diarrhea was significantly shorter than that in control group (P < 0.05). After treatment, the levels of CD4+, CD4+/CD8+, IgA, IgM and IgG in the observation group were higher than those in the control group, and the levels of CD8+ were lower than those in the control group, with statistical significance (P < 0.05). There was no statistical significance in the levels of blood indexes above between the two groups before treatment (P > 0.05). Conclusion: Early administration of Saccharomyces boulardii can effectively reduce the risk of AAD, shorten the duration of diarrhea and improve immune func-tion in children with pneumonia treated with antibiotics.
