摘要: 糖尿病足溃疡造成的足部感染、化脓、溃疡等症状可导致溃疡面长期不愈、截肢甚至死亡,是严重影响病人身心健康的并发症之一。糖尿病足溃疡的发病机制和临床表现复杂,目前尚缺乏早期诊断和评估病情严重程度的生物标志物。间充质干细胞来源的外泌体参与细胞间通讯,通过调节炎症、血管再生、加速细胞上皮化和细胞外基质重塑等机制在糖尿病创面修复过程中发挥作用。此外,基于聚合物的伤口敷料支架可以将外泌体输送至所需的靶点,成为糖尿病创面精准治疗的创新靶点。本文综述了外泌体在糖尿病创面愈合中的作用机制及在诊断治疗中的潜在应用。
Abstract: The symptoms of foot infection, suppuration and ulceration caused by diabetic foot ulcers can lead to long-term non-healing of the ulcerated surface, amputation and even death, which is one of the complications that seriously affect the physical and mental health of patients. The pathogenesis and clinical manifestations of diabetic foot ulcers are complex, and there is currently a lack of bi-omarkers for early diagnosis and assessment of the severity of the disease. Mesenchymal stem cell-derived exosomes participate in intercellular communication and play a role in diabetic wound repair by regulating inflammation, angiogenesis, accelerating cell epithelialization and extracellu-lar matrix remodeling. In addition, polymer-based wound dressing scaffolds can deliver exosomes to the desired target, becoming an innovative target for the precision treatment of diabetic wounds. This article reviews the mechanism of exosomes in the healing of diabetic wound and their potential application in diagnosis and treatment.
