腹壁上动脉灌注化疗治疗局部晚期乳腺癌1例

Superior Epigastric Arterial Infusion Chemotherapy for Locally Advanced Breast Cancer: A Case Report

摘要: 局部晚期乳腺癌(Locally Advanced Breast Cancer, LABC)通常指IIB或IIIA/B/C期。约占新诊断乳腺癌的5%，相较于不发达的国家，发达国家LABC在患者中的比例更低。LABC具有肿块进展迅速、侵袭转移能力强、且预后差，无法尽快手术切除等特点。目前，新辅助化疗是指南中对局部晚期乳腺癌治疗的首选方式，但仍存在新辅助化疗效果不佳的现况，且一部分高龄或体质较差的患者不能耐受全身化疗。动脉灌注化疗(Arterial Infusion Chemotherapy, AIC)的治疗方式是直接将化疗药物通过肿瘤的滋养动脉灌注到肿瘤组织，使肿瘤组织坏死。AIC副作用较小，可以使全身化疗效果不佳、甚至进展的患者达到肿瘤降期的目的。现将我科诊治一例新辅助化疗效果不佳患者，给予腹壁上动脉灌注治疗予以总结。

Abstract: Locally advanced breast cancer (LABC) is usually referred to as stage IIB or IIIA/B/C. It accounts for about 5% of newly diagnosed breast cancer. Compared with less developed countries, the proportion of LABC in patients is lower in developed countries. LABC has the characteristics of rapid progression, strong invasion and metastasis, and poor prognosis, which cannot be surgically removed as soon as possible. Currently, neoadjuvant chemotherapy is the first choice for locally advanced breast cancer according to the guidelines. However, the effect of neoadjuvant chemotherapy is still poor, and some patients with old age or poor physique can not tolerate systemic chemotherapy. Arterial Infusion Chemotherapy (AIC) is a therapeutic method in which chemotherapy drugs are directly infused into the tumor tissue through the nutrient arteries of the tumor to cause tumor tissue necrosis. AIC has less side effects, and patients with poor response to systemic chemotherapy or even progression can achieve the goal of tumor downstaging. In this paper, a patient with poor effect of neoadjuvant chemotherapy in our department was treated with superior epigastric arterial infusion.
