老年人家庭医生签约服务现状及问题调查——以太原市杏花岭区D社区卫生服务中心为例
Survey on the Current Situation and Problems of Contracted Family Doctor Services for the Elderly—Taking D Community Health Service Center of Xinghualing District, Taiyuan City as an Example
摘要: 随着社会老龄化程度的加深,老年人的健康问题不仅关乎个人和家庭,也关系到社会的和谐稳定,家庭医生签约服务作为一种有效的健康管理方式,有助于提升老年人的健康水平和生活质量,还有助于优化医疗资源配置,促进社会的和谐发展。本文从需方和供方两个角度出发,利用问卷调查法和访谈法对太原市杏花岭区D社区卫生服务中心老年人群体和家庭医生团队进行调查研究。研究发现,首先,老年人对于家庭医生签约服务的相关政策的认识程度不高,且较为看重家庭医生的技术水平,大部分老年人签约是由于别人推荐或为了方便,而对家庭医生政策不了解、不知道怎么签约和认为自己身体健康则是老年人未签约的原因;其次,签约老年人获得最多的服务为健康体检和健康教育,门诊就诊、上门就诊、电话咨询是老年人最希望的服务提供形式;同时,家庭医生数量少、质量低,工作积极性差,家庭医生签约服务利用率低,相关政策与制度缺失也是阻碍老年人家庭医生签约服务发展的主要问题。针对上述问题,本文提出了相应的对策建议:加大宣传力度,拓宽沟通渠道;加强人才建设,提高服务能力;提高资金投入,重视精神激励;完善管理机制,加大政策倾斜。
Abstract:
With the deepening of the aging of society, the health problems of the elderly are not only related to individuals and families, but also related to the harmony and stability of society. As an effective health management method, family doctor contract service is conducive to improving the health level and quality of life of the elderly, optimizing the allocation of medical resources and promoting the harmonious development of society. In this paper, the elderly group and the family doctor team in D community health service center of Xinghualing District of Taiyuan City were investigated by questionnaire survey and interview from both sides. The study found that, first of all, the elderly do not have a high understanding of the relevant policies of family doctors to sign services, and pay more attention to the technical level of family doctors. Most of the elderly sign contracts because of the recommendation of others or for convenience, while the elderly do not understand the policies of family doctors, do not know how to sign contracts and think that they are healthy are the reasons why they do not sign contracts. Secondly, the contracted elderly get the most services for health examination and health education, and outpatient visits, home visits, telephone consultation are the most desirable forms of service provision for the elderly; At the same time, the number of family doctors is small, the quality is low, the work enthusiasm is poor, the utilization rate of family doctor contract service is low, and the lack of relevant policies and systems are also the main problems hindering the development of family doctor contract service for the elderly. In view of the above problems, this paper puts forward the corresponding countermeasures and suggestions: strengthen the publicity, broaden the channel of communication; strengthen the construction of talents and improve the service ability; increase capital investment and pay attention to spiritual stimulation; improve management mechanisms and give greater preference to policies.
