儿童烟雾病的临床及影像诊断技术进展

摘要: 烟雾病是颈内动脉及其分支进行性狭窄导致脑血流灌注降低及中风风险增加的一种进展性脑血管疾病。儿童烟雾病以脑缺血最为常见,也可见颅内出血、癫痫发作、认知障碍及高血压表现。TCD、CT及MRI检查可为儿童烟雾病的早期诊断提供线索,DSA、MRI/MRA作为确诊手段各有所长。对儿童脑血流动力学状态进行定量评估,可指导治疗及随访。
Abstract: Moyamoya disease is a progressive cerebrovascular disease in which progressive stenosis of the internal carotid artery and its branches leads to decreased cerebral blood perfusion and increased risk of stroke. Moyamoya disease in children is most common with cerebral ischemia, and intracranial hemorrhage, seizures, cognitive impairment, and hypertension are also seen. TCD, CT and MRI can provide clues for the early diagnosis of moyamoya disease in children, and DSA and MRI/MRA have their own advantages as diagnostic methods. Quantitative assessment of the child’s cerebral hemodynamic status can guide treatment and follow-up.
