细胞外囊泡与自噬的串扰作用在骨性关节炎中的应用

摘要: 骨关节炎(OA)是最常见的慢性关节疾病,患病率随着年龄的增长而增加,并影响到大多数65岁以上的人。影响超过2.4亿人,是致残的主要原因。其病理学特征是进行性软骨变性,软骨下骨重塑和骨冗余形成增加,对患者的家庭和社会构成巨大负担,虽然一些与OA相关的危险因素已被提出,包括遗传易感性、衰老、肥胖和关节排列不良,但由于对软骨基质降解和OA发生的确切分子机制知之甚少,目前除了药物干预与全关节置换手术外,尚无有效的干预措施来减缓OA的进展或延缓软骨的不可逆降解。越来越多的证据表明细胞外囊泡(Extracellular Vesicles, EVs)和自噬在OA的进展和治疗中发挥着重要作用,本文主要综述了细胞外囊泡与自噬之间的相互作用在治疗OA中的潜在治疗策略。
Abstract: Osteoarthritis (OA) is the most common chronic joint disease, increasing in prevalence with age and affecting the majority of people over the age of 65. Affecting more than 240 million people, it is a leading cause of disability. Its pathologic features are progressive cartilage degeneration, increased subchondral bone remodeling and bone redundancy formation, which pose a great burden to the patient’s family and society. Although a number of risk factors associated with OA have been proposed, including genetic predisposition, aging, obesity, and poor joint alignment, little is known about the exact molecular mechanisms underlying the degradation of cartilage matrix and the occurrence of OA. At present, except for drug intervention and total joint replacement, there is no effective intervention to slow the progression of OA or delay the irreversible degradation of cartilage. There is increasing evidence that extracellular vesicles (EVs) and autophagy play an important role in the progression and treatment of OA. This article mainly reviews the potential therapeutic strategies of the interaction between extracellular vesicles and autophagy in the treatment of OA.
