类风湿关节炎全膝置换1例报告

摘要: 类风湿关节炎(RA)是一种病因未明的慢性、以炎性滑膜炎为主的系统性疾病。其特征是手、足小关节的多关节、对称性、侵袭性关节炎症，血管翳的形成对关节软骨，软骨下骨，韧带和肌腱中的胶原基质的侵蚀作用，使关节腔破坏，上下面融合，发生纤维化性强硬、错位，甚至骨化，功能完全丧失，相近的骨组织也产生废用性的稀疏。因此，晚期类风湿关节炎采用保守药物治疗往往不能取得较好疗效。关节置换手术术中可以彻底清除病变软组织，重新恢复关节稳定性，恢复关节力线，是治疗类风湿关节炎的有效手段。
Abstract: Rheumatoid arthritis (RA) is a chronic systemic disease of unknown etiology, mainly inflammatory synovitis. It is characterized by polyarticular, symmetrical, and aggressive joint inflammation of the small joints of the hands and feet. The formation of pannus erodes the collagen matrix in the articular cartilage, subchondral bone, ligaments, and tendons, causing destruction of the joint cavity and upper and lower joints. Fusion, fibrotic stiffness, dislocation, and even ossification occur, complete loss of function, and disuse sparseness of adjacent bone tissue. Therefore, conservative drug treatment for advanced rheumatoid arthritis often fails to achieve good results. Joint replacement surgery can completely remove the diseased soft tissue, restore joint stability, and restore joint alignment. It is an effective method for treating rheumatoid arthritis.
