血尿酸水平对缺血性脑卒中患者入院病情程度的相关性分析

摘要: 目的:探讨血清尿酸水平对急性缺血性脑卒中患者入院病情程度的影响。方法:回顾性收集临沂市人民医院神经内科2021年9月~2022年6月收治的277例急性缺血性脑卒中患者的临床资料信息,依据美国国立卫生研究院卒中评分量表(NIHSS)将患者分为轻度卒中(评分 ≤ 4分)和中重度卒中(评分 > 4分),比较2组患者临床差异,将单因素分析得出有意义的指标进一步纳入二元Logistic回归分析中,筛选出影响患者病情程度的独立影响因素。结果:277例病人中轻度脑卒中188例,中重度脑卒中89例。与轻度脑卒中相比,中重度脑卒中患者房颤病史比例较高,而高脂血症病史比例及血清尿酸水平较低,差异具有统计学意义(P < 0.05)。其中血尿酸(OR = 0.996, 95%CI: 0.993~0.999, P = 0.003)、房颤病史(OR = 0.149, 95%CI: 0.028~0.795, P = 0.026)是病情程度的独立影响因素。结论:正常范围内较高的血尿酸水平使入院缺血性脑卒中患者病情较轻。
Abstract: Objective: To investigate the effect of serum uric acid level on admission degree of acute ischemic stroke patients. Methods: The clinical data of 277 patients with acute ischemic stroke admitted to the Department of Neurology of Linyi People’s Hospital from September 2021 to June 2022 were retrospectively collected. According to the National Institutes of Health Stroke Score Scale (NIHSS), the patients were divided into mild stroke (score ≤ 4 points) and moderate and severe stroke (score > 4 points). The clinical differences between the two groups were compared, and the mean-ingful indicators obtained from the univariate analysis were further incorporated into the binary Logistic regression analysis to screen out the independent influencing factors affecting the severity of the patients’ disease. Results: Among the 277 patients, 188 had mild stroke and 89 had moder-ate and severe stroke. Compared with mild stroke, patients with moderate to severe stroke had a higher proportion of atrial fibrillation history, while patients with hyperlipidemia history and se-rum uric acid content were lower, and the difference was statistically significant (P < 0.05). Among them, blood uric acid (OR = 0.996, 95%CI: 0.993~0.999, P = 0.003) and history of atrial fibrillation (OR = 0.149, 95%CI: 0.028~0.795, P = 0.026) were independent factors influencing the severity of the disease. Conclusion: Higher uric acid level in the normal range can make the patients with is-chemic stroke less ill.
