质子泵抑制剂四联疗法联合双歧杆菌四联活菌对幽门螺杆菌感染伴胃溃疡的疗效

Clinical Efficacy of Proton Pump Inhibitor Quadruple Therapy Combined with Bifidobacterium Quadruple Live Bacterial Tablets in the Treatment of Helicobacter pylori Infection with Gastric Ulcer

摘要: 目的：探讨质子泵抑制剂四联疗法联合双歧杆菌四联活菌治疗幽门螺杆菌(Hp)感染阳性伴胃溃疡的临床效果。方法：回顾性分析2020年1月至2023年12月收治的88例Hp阳性伴胃溃疡患者，随机分为常规组和治疗组，各44例，常规组采用质子泵抑制剂四联疗法，治疗组加用双歧杆菌四联活菌治疗。观察对比两组胃溃疡预后、Hp根除率、胃黏膜愈合修复前后评分、血清胃肠激素(胃泌素、胃动素、生长抑素)水平及不良反应情况。结果：经分析，治疗组胃溃疡愈合总体有效率、Hp根除率高于常规组(93.18% vs 81.82%, 72.72% vs 56.81%, P < 0.05)；治疗组胃黏膜损害修复评分、血清胃肠激素(水平胃泌素、胃动素、生长抑素)改善程度均优于常规组(P < 0.05)；两组间不良反应发生率无统计学差异(11.36% vs 15.90%, P = 0.56)。结论：质子泵抑制剂四联疗法联合双歧杆菌四联活菌对Hp感染阳性伴胃溃疡患者的溃疡愈合疗效显著，Hp根除率高，且安全有效。

Abstract: Objective: Exploring the clinical efficacy of proton pump inhibitor quadruple therapy combined with Bifidobacterium quadruple live bacterial tablets in the treatment of Helicobacter pylori (Hp) positive gastric ulcers. Method: A retrospective analysis was conducted on 88 Hp positive gastric ulcer patients admitted from January 2020 to December 2023. They were randomly divided into a control group and a treatment group, with 44 cases in each group. The control group received proton pump inhibitor quadruple therapy, while the treatment group received treatment with Bifidobacterium quadruple live bacterial tablets, observing and comparing the prognosis of gastric ulcers, Hp eradication rate, healing and repair of gastric mucosal injury, serum levels of gastrointestinal hormones (gastrin, motilin, somatostatin), and adverse reactions between the two groups. Result: After analysis, the overall effective rate and Hp eradication rate of gastric ulcer healing in the treatment group were higher than those in the conventional group (93.18% vs 81.82%, 72.72% vs 56.81%, P < 0.05); the treatment group showed better improvement in gastric mucosal damage repair score and serum gastrointestinal hormones (levels of gastrin, motilin, and somatostatin) compared to the conventional group (P < 0.05); There was no statistically significant difference in the incidence of adverse reactions between the two groups (11.36% vs 15.90%, P = 0.56). Conclusion: The combination of proton pump inhibitor quadruple therapy and Bifidobacterium quadruple live bacterial tablets has a significant therapeutic effect on ulcer healing in Hp positive gastric ulcer patients, with a high Hp eradication rate, and is safe and effective.
