团体心理辅导对炎症性肠病患者自我概念及心理状况影响的混合研究

摘要: 目的：分析团体心理辅导在炎症性肠病患者中的应用效果。方法：选取2022年9月至2023年1月江苏省某三甲医院消化内科炎症性肠病患者，采用随机数字表法分为对照组和干预组，各18例，干预组在对照组干预措施的基础上增加以心理剧为主体的团体心理辅导，对照组接受消化内科常规护理干预并在出院后建立微信群进行健康宣教，干预时长均为8周。比较两组干预前后自我概念量表评分、心理弹性量表评分、焦虑自评量表评分及睡眠状况自评量表评分的差异；对干预组18名患者参与团体心理辅导的感受分享采用Colaizzi 7步分析法分析、归纳、提炼资料。结果：干预前，两组自我概念量表评分、心理弹性量表评分、焦虑自评量表评分及睡眠状况自评量表评分比较，差异无统计学意义(P > 0.05)；干预后，干预组自我概念量表评分、心理弹性量表评分均高于对照组(P > 0.05)，焦虑自评量表评分、睡眠状况自评量表评分均低于对照组(P > 0.05)；提炼出重塑自我、角色扩展、自我实现3个主题。结论：团体心理辅导可以提升炎症性肠病患者的自我概念，改善心理状态。
Abstract: Objective: This paper aims to analyze the application effect of group psychological counseling in patients with inflammatory bowel disease. Method: Patients with inflammatory bowel disease in the gastroenterology department of a tertiary hospital in Jiangsu Province from September 2022 to January 2023 were selected and randomly divided into a control group and an intervention group using a random number table method, with 18 cases in each group. The intervention group received group psychological counseling with psychodrama as the main body on the basis of the intervention measures in the control group. The control group received routine nursing intervention in the gastroenterology department and established a WeChat group for health education after discharge. The intervention duration was 8 weeks for both groups. the differences in self-concept scale scores, CD-RISC scores, SAS scores, and SRSS scores between the two groups before and after intervention were compared; The experience sharing of 18 patients in the intervention group participating in group psychological counseling was analyzed, summarized, and extracted using the Colaizzi 7-step analysis method. Results: Before the intervention, there was no statistically significant difference between the two groups in terms of self-concept scale scores, CD-RISC scores, SAS scores, and SRSS scores (P > 0.05); After intervention, the self-concept scale scores and CD-RISC scores of the intervention group were higher than those of the control group (P > 0.05), while the SAS scores and SRSS scores were lower than those of the control group (P > 0.05); Three themes were extracted: reshaping oneself, expanding roles, and achieving oneself. Conclusion: Group psychological counseling can enhance the self-concept of patients with inflammatory bowel disease and improve their psychological state.
