介入治疗左下腔静脉畸形合并右髂静脉压迫综合征一例

Interventional Treatment of Left Inferior Vena Cava Malformation Complicated with Right Iliac Vein Compression Syndrome: A Case Report

摘要: 通过对1例左下腔静脉畸形合并右髂静脉受压同时伴有右肾静脉开口变异及双下肢深静脉血栓形成的青年女性患者的病例报道，回顾患者的诊疗过程，查阅相关文献，了解该类疾病的临床特征、影像学表现、诊断及治疗进展。发现对于右下肢深静脉血栓形成患者，应考虑到左下腔静脉畸形可能。此类疾病治疗上除规范抗凝外，可能需要更加积极的手术干预才能尽快改善症状恢复血流通畅，同时植入髂静脉支架可能比单纯球囊扩张具有更高的中远期通畅率。

Abstract: Based on the case report of a young female patient with left inferior vena cava malformation complicated with right iliac vein compression accompanied by right renal vein opening variation and deep vein thrombosis of both lower limbs, we reviewed the diagnosis and treatment of her process, consulted relevant literature, aiming at understanding the clinical characteristics, imaging manifestations, diagnosis and treatment progress of this disease. It was found that for patients with right lower extremity deep vein thrombosis, the possibility of left inferior vena cava malformation was considered. In the treatment of such diseases, in addition to standard anticoagulation, more active surgical intervention may be required to improve symptoms and restore blood flow patency as soon as possible. Meanwhile, the implantation of iliac vein stents in one stage may have a higher patency rate in the medium and long term than that of balloon dilation alone.
