基于“脏腑别通”理论从心胆论治代谢相关性脂肪肝病伴焦虑抑郁

摘要: 代谢相关性脂肪肝病(Metabolic-associated fatty liver disease, MAFLD)患者的精神健康问题越来越受到关注，在中医理论中，心胆调节情志，心胆失调不仅与焦虑、抑郁等情志疾病相关，也可能影响代谢性疾病的发生发展，尤其是代谢相关性脂肪性肝病，故以“脏腑别通”理论为基础，从心胆脏腑相通、经络相连为切入点，揭示焦虑抑郁状态下的代谢相关性脂肪肝病的中医基本内涵，提出“心胆同治，调肝安神”之治法，通过调理心胆气机，缓解焦虑抑郁情志障碍，拓展MAFLD伴焦虑抑郁的临床诊疗思路，为身心兼治的综合治疗方案提供中医理论支持与实践依据。
Abstract: Mental health problems of patients with Metabolic-associated fatty liver disease (MAFLD) have attracted more and more attention. In the theory of traditional Chinese medicine, heart and gallbladder regulate emotions, and heart and gallbladder disorders are not only related to emotional diseases such as anxiety and depression, but also related to emotional diseases. It may also affect the occurrence and development of metabolic diseases, especially metabolism-related fatty liver disease. Therefore, based on the theory of “Zangfu Bietong” and starting from the heart-gallbladder and viscera connection and meridians connection, the basic connotation of traditional Chinese medicine of metabolism-related fatty liver disease in the state of anxiety and depression is revealed, and the treatment method of “heart-gallbladder combined treatment, regulating the liver and soothing the mind” is proposed. Alleviate anxiety and depression emotional disorders, expand the clinical diagnosis and treatment ideas of MAFLD combined with anxiety and depression, and provide theoretical support and practical basis for the comprehensive treatment plan of body and mind.
