摘要: 目的:通过俞募配穴对功能性便秘的临床观察以及对血清SP (Substance P)和VIP (Vasoactive Intestinal Peptide)水平的影响,结合“脑–肠轴”理论来探讨俞募配穴治疗功能性便秘的作用机制。方法:将142例功能性便秘患者随机分为俞募配穴针刺组(71例)、非经非穴组(71例)。治疗前后进行自发性排便(SBM)、大便性状评分(BSFS)、排便困难程度评定,并使用血清酶联免疫吸附试验(ELISA)测定血清中SP和VIP含量。结果:俞募配穴组自发性排便(SBM)、大便性状评分(BSFS)、排便困难程度等均优于非经非穴组,且俞募配穴组患者血清VIP含量低于非经非穴组,血清SP含量高于非经非穴组,皆有统计学意义(P < 0.05)。结论:俞募配穴治疗功能性便秘效果突出,能显著缓解便秘症状。而针刺俞募配穴可以通过减少VIP含量、增加血清SP含量,调节脑–肠轴功能,缓解功能性便秘临床表现。
Abstract: Objective: Through the clinical observation on functional constipation and the effect on serum SP (Substance P) and VIP (Vasoactive Intestinal Peptide) levels of back-shu points and front-mu points, combined with the theory of “Brain-Intestinal Axis” to explore the effect mechanism was investigated with the theory of “brain-intestinal axis”. Methods: 142 patients with functional constipation were randomly divided into the acupuncture group (71 patients) and the non-meridian and non-acupuncture group (71 patients). Before and after the treatment, spontaneous bowel movement (SBM), stool character score (BSFS), and the degree of defecation difficulty were evaluated, and the serum enzyme-linked immunosorbent assay (ELISA) was used to determine the levels of SP and VIP in serum. Results: Spontaneous Bowel Movement (SBM), Bowel Size Function Score (BSFS), and Difficulty in Bowel Movement were better in the back-shu points and front-mu points combination group than in the non-menstruation and non-point group, and the serum VIP level was lower than that of the non-menstruation and non-point group, and the serum SP level was higher than that of the non-menstruation and non-point group, which were all statistically significant (P < 0.05). Conclusion: The effect of back-shu points and front-mu points combination in treating functional constipation is outstanding, and it can significantly relieve the symptoms of constipation. Acupuncture on back-shu points and front-mu points combination can regulate the function of brain-intestinal axis and alleviate the clinical manifestations of functional constipation by decreasing the content of VIP and increasing the content of serum SP.
