通心络胶囊联合替罗非班治疗急性心肌梗死的临床研究

摘要: 目的:探讨通心络胶囊联合替罗非班治疗急性心肌梗死的临床疗效。方法:选取2021.09~2024.09本院收治得到118例急性心肌梗死患者为对象,以信封抽签法分为观察组、对照组,每组59例。对照组以替罗非班治疗,观察组联合通心络胶囊治疗。评估两组临床疗效、心功能、心肌损伤标志物、氧化应激、血管内皮功能和不良反应发生情况。结果:观察组总有效率高于对照组(P < 0.05)。治疗后,观察组心功能指标左室射血分数(LVEF)、每搏输出量(SV)、心输出量(CO)、心指数(CI)水平高于对照组(P < 0.05)。治疗后,观察组心型脂肪酸结合蛋白(H-FABP)、心肌肌钙蛋白I (cTnI)、氨基末端脑利钠肽前体(NT-proBNP)均低于对照组(P < 0.05)。治疗后,观察组丙二醛(MDA)、髓过氧化物酶(MPO)低于对照组,超氧化物歧化酶(SOD)高于对照组(P < 0.05)。治疗后,观察组一氧化氮(NO)、血管内皮生长因子(VEGF)均高于对照组,血管内皮素-1 (ET-1)低于对照组(P < 0.05)。两组不良反应发生率无差异性(P > 0.05)。结论:通心络胶囊联合替罗非班治疗急性心肌梗死的临床疗效显著,可提升患者心功能,降低心肌损伤标志物水平,抑制机体的氧化应激反应水平,改善患者的血管内皮功能,治疗安全性良好。
Abstract: Objective: To investigate the clinical efficacy of Tongxinluo capsule combined with Tirofiban in the treatment of acute myocardial infarction. Methods: A total of 118 patients with acute myocardial infarction were selected from 2021.09 to 2024.09. They were divided into observation group and control group by envelope drawing method, with 59 cases in each group. The control group was treated with tirofiban, and the observation group was treated with Tongxinluo capsule. Clinical efficacy, cardiac function, markers of myocardial injury, oxidative stress, vascular endothelial function and occurrence of adverse reactions were evaluated in both groups. Results: The total effective rate of observation group was higher than that of control group (P < 0.05). After treatment, the levels of left ventricular ejection fraction (LVEF), stroke output (SV), cardiac output (CO) and cardiac index (CI) in observation group were higher than those in control group (P < 0.05). After treatment, cardiac fatty acid-binding protein (H-FABP), cardiac troponin I (cTnI) and amino terminal brain natriuretic peptide precursor (NT-proBNP) in observation group were lower than those in control group (P < 0.05). After treatment, malondialdehyde (MDA) and myeloperoxidase (MPO) in observation group were lower than those in control group, and superoxide dismutase (SOD) was higher than those in control group (P < 0.05). After treatment, nitric oxide (NO) and vascular endothelial growth factor (VEGF) in observation group were higher than those in control group, and vascular endothelin-1 (ET-1) was lower than those in control group (P < 0.05). There was no difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion: Tongxinluo capsule combined with Tirofiban has significant clinical efficacy in the treatment of acute myocardial infarction, which can improve the sexual function of patients, reduce the level of myocardial injury markers, inhibit the level of oxidative stress response, improve the vascular endothelial function of patients, and has good therapeutic safety.
