Abstract: Super-refractory status epilepticus (SRSE) is a life-threatening neurological emergency associated with high mortality and disability rates. Traditional therapeutic approaches, including anti-epileptic drugs and anesthetic infusions, often yield suboptimal outcomes. Recent advances in epileptic network theory and neuroelectrophysiology have established neuromodulation techniques as a critical component of multimodal therapeutic strategies for SRSE. This review systematically evaluates clinical evidence for invasive techniques—vagus nerve stimulation (VNS), deep brain stimulation (DBS), and responsive neurostimulation (RNS)—and non-invasive modalities, including transcranial magnetic stimulation (TMS), electroconvulsive therapy (ECT), and transcranial direct current stimulation (tDCS). By elucidating the therapeutic efficacy of these neuromodulation approaches, we aim to guide the development of individualized treatment protocols, optimize seizure control rates and neurological functional recovery, and ultimately improve long-term quality of life for SRSE patients.
