Abstract: The present review examines the current landscape and emerging developments of MRI-derived scoring systems for characterizing adnexal masses. Firstly, the background of the ADNEX MR SCORING system and the O-RADS MRI scoring system were reviewed, and their diagnostic performance and clinical value for indeterminate adnexal masses under ultrasound were reviewed, respectively. Secondly, the classification criteria of the two MRI scoring systems and the related descriptive terms of MRI signs are organized. The clinical progress and limitations of the two scoring systems in recent years are then summarized, and finally, the possible future directions of MRI scoring systems are further explored and discussed. The aim of this study is to provide some feasible ideas and suggestions for subsequent studies on the preoperative evaluation and accurate classification of adnexal masses.
