Abstract: Breast cancer is the most common malignant tumor among women worldwide. Although chemotherapy serves as a critical treatment modality and effectively controls disease progression, it often induces adverse effects such as negative emotions significantly impairing patients’ quality of life and treatment adherence. Walking exercise, as a cost-effective and accessible intervention, has demonstrated significant potential in improving the psychological well-being of breast cancer patients undergoing chemotherapy. However, relevant domestic research remains limited. This systematic review examines the efficacy, underlying mechanisms, and clinical applicability of walking exercise in managing negative emotions among this patient population, while highlighting current research limitations. The findings provide evidence-based support for standardizing the implementation of walking exercise in emotional management for breast cancer patients undergoing chemotherapy in China.
