心电图动态演变为Aslanger征急性心肌梗死病例1例

A Case of Acute Myocardial Infarction with Dynamic Electrocardiographic Evolution to Aslanger Pattern

摘要: 诊断ST段抬高性心肌梗死时，要求心电图上有连续2个及以上的导联ST段抬高，这会导致一些冠状动脉急性闭塞患者不符合这一条件而延误治疗，影响预后。近年来，单导联ST段抬高的下壁心梗心电图Aslanger征被广泛认识，为病人争取到了早期再灌注的机会。本文报告1例心电图动态演变为Aslanger征，经冠状动脉造影证实为胸痛患者伴有剩余血管狭窄的回旋支闭塞，以进一步提高临床对此心电图表现的认识。

Abstract: The diagnosis of ST-segment elevation myocardial infarction (STEMI) requires ST-segment elevation in two or more contiguous leads on the electrocardiogram (ECG). This criterion may lead to delayed treatment and poorer prognosis in patients with acute coronary occlusion who do not meet these conditions. In recent years, the Aslanger pattern—a single-lead ST-segment elevation observed in inferior myocardial infarction—has gained widespread recognition, enabling early diagnosis and timely reperfusion therapy. This article presents a case of a patient with chest pain whose ECG dynamically evolved into the Aslanger pattern, later confirmed by coronary angiography as occlusion of the circumflex artery with stenosis in other vessels. The report aims to enhance clinical awareness of this ECG manifestation.
