摘要: 目的:探讨不同浓度医用臭氧联合甲钴胺、利多卡因治疗带状疱疹后遗神经痛(Postherpetic neuralgia, PHN)患者的临床效果。方法:选取2024年7月至2025年7月与内蒙古自治区人民医院行带状疱疹后遗神经痛治疗的病人60例患者进行研究。分为低浓度组(25 μg/mL臭氧)、中浓度组(30 μg/mL臭氧)、高浓度组(35 μg/mL臭氧)各20例,所有病人均采用超声引导下臭氧联合利多卡因、甲钴胺进行胸椎旁间隙注射治疗。比较三组患者VAS、PSQI评分及临床效果。结果:治疗后,中浓度组和高浓度组的睡眠质量评分、治疗显效率均优于低浓度组;中浓度组和高浓度组治疗20 min后VAS评分均优于低浓度组;中、高浓度组的睡眠质量评分、治疗有效率、VAS评分差异无统计学意义。治疗后三组患者使用辅助药物概率及不良反应发生率差异无统计学意义。结论:30 μg/mL臭氧浓度联合甲钴胺、利多卡因治疗PHN患者效果理想,再增加臭氧浓度对提高治疗效果作用并不显著。
Abstract: Objective: To investigate the clinical efficacy of different concentrations of medical ozone combined with mecobalamin and lidocaine in the treatment of patients with postherpetic neuralgia (PHN). Methods: A total of 60 patients with PHN treated at the People’s Hospital of Inner Mongolia Autonomous Region from July 2024 to July 2025 were selected for the study. They were divided into low-concentration group (25 μg/mL ozone), medium-concentration group (30 μg/mL ozone), and high-concentration group (35 μg/mL ozone), with 20 cases in each group. All patients received ultrasound-guided parathoracic spinal space injection therapy with ozone combined with lidocaine and mecobalamin. The Visual Analogue Scale (VAS), Pittsburgh Sleep Quality Index (PSQI) scores, and clinical efficacy were compared among the three groups. Results: After treatment, the sleep quality scores and marked effective rates in the medium-concentration and high-concentration groups were superior to those in the low-concentration group. The VAS scores at 20 minutes after treatment in the medium-concentration and high-concentration groups were better than those in the low-concentration group. There were no statistically significant differences in sleep quality scores, effective rates, and VAS scores between the medium-concentration and high-concentration groups. There were no statistically significant differences in the probability of using adjuvant drugs and the incidence of adverse reactions among the three groups after treatment. Conclusion: A 30 μg/mL ozone concentration combined with mecobalamin and lidocaine has an ideal effect in the treatment of PHN patients, and increasing the ozone concentration does not significantly improve the therapeutic effect.
