Shaoyang Marrow-Nourishing and Spirit-Regulating Therapy in the Treatment of Myelodysplastic Syndromes

摘要: 本文章基于“少阳主枢，通达三焦”气化理论，结合“精充髓海，形神得养”学术思想，构建“枢机–髓海–神机”三维辨治体系。提出骨髓增生异常综合征的核心病机在于少阳枢机失运，三焦气化失司导致肾精化髓无权，引发“髓海失充–气血乖逆–神机失调”的恶性传变。治疗遵循“调畅气机以复生化之本，培补精髓以充髓海之源，燮理阴阳以固神机之本”的序贯原则：首重疏利少阳枢机，运用升降相因之法恢复三焦气化职能；次施补肾填精，通过肝肾同调促进精血互化以充盈髓海；终以形神同治，运用动静结合疗法调节脏腑气机平衡。该体系有机融合气机调畅理论与虚损类病治则，阐明“气化有权则精髓自生，髓海得充则神机乃复”的中医治髓理念，为现代难治性血液病提供中医原创思维的系统诊疗范式。

Abstract: This article innovatively constructs a tripartite diagnostic and therapeutic system centered on the “Pivot (Shu Ji)-Marrow Sea (Sui Hai)-Spiritual Mechanism (Shen Ji).” It is based on the qi transformation theory of “Shaoyang governs the pivot, regulating Sanjiao (Triple Burner)” and integrates the academic concept of “essence replenishes the marrow sea, nurturing the body and spirit.” The core pathogenesis of myelodysplastic syndrome (MDS) is proposed as follows: dysfunction of the Shaoyang pivotal mechanism and disordered Sanjiao qi transformation impair the kidney’s ability to generate marrow from essence. This triggers a vicious cycle of “marrow sea depletion-qi-blood derangement-spiritual mechanism imbalance.” Treatment follows a sequential principle: at first, “regulating qi movement to restore the foundation of biochemical production”: Prioritize unblocking the Shaoyang pivot using ascending-descending coordination to normalize Sanjiao qi transformation. Secondly, “tonifying essence to replenish the marrow sea”: Strengthen the kidney and supplement essence, promoting essence-blood mutual transformation through liver-kidney synergy to enrich the marrow reservoir. Finally, “harmonizing yin-yang to stabilize the spiritual mechanism’s foundation”: Employ form-spirit co-regulation using dynamic-static integrated therapies to balance the visceral qi dynamics. This system organically integrates qi regulation theories with therapeutic principles for deficiency-exhaustion disorders. It elucidates the fundamental TCM concept for treating marrow disorders: “Functional qi transformation spontaneously generates essence and marrow; a replenished marrow sea restores the spiritual mechanism”. It thereby provides a systematic TCM diagnostic and therapeutic paradigm, rooted in original Chinese medical thinking, for modern refractory hematologic diseases.
