一种基于中医辨证施治的中药临方制剂复方大黄膏对重度混合痔的疗效研究

A Study on the Efficacy of Compound Dahuang Ointment, a TCM Prescribed Preparation Based on Syndrome Differentiation and Treatment in the Treatment of Severe Mixed Hemorrhoids

摘要: 目的：观察湿热型临方制剂——复方大黄膏在治疗重度混合痔中的临床疗效。方法：将70例湿热型混合痔四期患者随机分为治疗组(n = 35)和对照组(n = 35)；治疗组给予湿热型临方制剂复方大黄膏外用，对照组给予马应龙麝香痔疮膏外用，所有患者均于就诊后第1天开始用药，疗程为10天。用药第5、10天观察两组患者的主要临床症状，比较两组的临床疗效。结果：用药第5、10天，治疗组患者的肛缘水肿情况优于对照组(P < 0.05)；用药第5、10天，治疗组患者的创面出血情况优于对照组(P < 0.05)；用药后第5、10天，治疗组患者的疼痛情况优于对照组(P < 0.05)。治疗后，治疗组和对照组的总有效率分别为100%和97.1%，显效率分别为62.9%和14.3%，治疗组疗明显效优于对照组。结论：湿热型混合痔四期的患者应用湿热型临方制剂复方大黄膏可有效缓解患者肛周疼痛、水肿、出血等症状，减轻患者的痛苦。

Abstract: Aim: To evaluate the clinical efficacy of the dampness-heat type formulation of Compound Rhubarb Ointment in patients with severe mixed hemorrhoids of dampness-heat pattern. Methods: Seventy patients with stage IV mixed hemorrhoids of dampness-heat pattern were randomly assigned to either the treatment group (n = 35) or control group (n = 35). The treatment group received topical Compound Rhubarb Ointment, while the control group received topical Mayinglong Musk Hemorrhoid Ointment. Treatment commenced on day 1 post-enrollment and continued for 10 days. Primary clinical symptoms were assessed at days 5 and 10, with intergroup efficacy comparisons conducted. Results: At days 5 and 10 post-treatment: Anal margin edema improvement was significantly greater in the treatment group (P < 0.05); Wound bleeding reduction was superior in the treatment group (P < 0.05); Pain relief was significantly better in the treatment group (P < 0.01 at day 5; P < 0.05 at day 10). After treatment, the total effective rates were 100% (treatment) vs. 97.1% (control), and marked effective rates were 62.9% vs. 14.3%, respectively, indicating superior efficacy in the treatment group (P < 0.05). Conclusion: Compound Rhubarb Ointment effectively alleviates pain, edema, and bleeding in patients with stage IV mixed hemorrhoids of dampness-heat pattern, demonstrating significant clinical benefits.
