双气囊小肠镜治疗胃石性小肠梗阻一例

A Case of Double-Balloon Enteroscopy for Treating Gastrolith-Induced Small Bowel Obstruction

摘要: 胃石性小肠梗阻临床少见，早期可无典型的临床表现，随着结石增大引起肠梗阻症状后被诊断明确。本文介绍我院一例高龄胃石性小肠梗阻患者的诊疗经过，患者初期行积极的内科药物保守治疗效果不佳，我科予以双气囊小肠镜碎石治疗后患者好转出院。本文对胃结石引起小肠梗阻的临床表现、病因、诊断、治疗进行总结，同时予以病例思考已期对消化内科医师提供临床参考。

Abstract: Gastrolith-induced small bowel obstruction (SBO) is a rare clinical entity. Early stages may lack typical clinical manifestations. The diagnosis is typically established subsequently to the development of SBO symptoms caused by the enlarging stone. This case report details the diagnosis and management of an elderly patient with gastrolith-induced SBO admitted to our hospital. Initial aggressive conservative medical therapy yielded suboptimal response. Subsequently, lithotripsy via double-balloon enteroscopy was performed by our department, leading to clinical improvement and eventual discharge of the patient. This article summarizes the clinical presentation, etiology, diagnosis, and management strategies of gastrolith-induced small bowel obstruction. Additionally, clinical insights derived from this case are provided to serve as a practical reference for gastroenterologists.
