Abstract: The woven coronary artery is an extremely rare type of congenital coronary artery malformation. The typical WCA presents with exertional chest pain or acute coronary syndrome, and its imaging features include a “twisted braid-like” multi-chambered separated structure in the coronary artery, often accompanied by congenital abnormalities of the same side coronary artery or thrombosis related to hypercoagulable state. The clinical manifestations can be asymptomatic or mild symptoms, or they can present as chest pain, myocardial infarction, etc. This article reports a case of a patient with WCA presenting mainly with chest pain. Through the reflection of the diagnosis and treatment of this case and the review of literature, it is expected to provide reference value for the diagnosis and treatment of woven coronary arteries.
