Study on the Prognostic Effect of ERAS in Elderly Patients Undergoing Total Hip Replacement during the Perioperative Period

摘要: 目的：将老年全髋置换术(THA)患者围术期增强术后恢复(eras)方案系统化、具体化后应用于老年初次单侧THA患者中，观察其在改善患者临床结局及降低医疗资源方面的效果。方法：选取2023年9月~2024年2月在重庆市大足区中医院首次单侧THA的患者，按照随机数字表法分试验组和对照组，对照组采用常规围手术期临床护理，试验组围手术期采用ERAS干预措施。收集试验组和对照组研究对象的VAS疼痛评分、Harris髋关节评分、日常生活活动能力评分(ADL)比较两组患者住院天数、住院总费用、满意度评分比较等，比较两组之间差异，分析ERAS在THA围手术期中的临床效果。结果：治疗前后VAS疼痛评分、Harris髋关节评分、ADL评分、两组患者住院天数、住院总费用、满意度评分比较，试验组均优于对照组(P < 0.05)。结论：ERAS在老年全髋置换术患者围术期的预后效果显著，值得临床推广并使用。

Abstract: Objective: To systematize and specify the enhanced postoperative recovery (ERAS) protocol during the perioperative period for elderly patients undergoing total hip arthroplasty (THA), and apply it to elderly patients with primary unilateral THA to observe its effect on improving clinical outcomes and reducing medical resources. Methods: Patients who underwent unilateral THA for the first time at Dazu District Hospital of Traditional Chinese Medicine in Chongqing from September 2023 to February 2024 were selected and divided into the experimental group and the control group according to the random number table method. The control group received conventional perioperative clinical care, while the experimental group received ERAS intervention measures during the perioperative period. The VAS pain score, Harris hip score, and activities of daily living (ADL) score of the experimental group and the control group were collected. The length of hospital stay, total hospitalization cost, and satisfaction score of the two groups were compared. The differences between the two groups were compared, and the clinical effect of ERAS during the THA operation period was analyzed. Results: Before and after treatment, comparisons of VAS pain score, Harris hip score, ADL score, length of hospital stay, total hospitalization cost, and satisfaction score between the two groups showed that the experimental group was superior to the control group (P < 0.05). Conclusion: ERAS has a significant prognostic effect in elderly patients undergoing total hip replacement during the perioperative period and is worthy of clinical promotion and application.
