摘要: 脊柱椎管内痛风结石是尿酸盐结晶沉积于椎管内的罕见病症,属于痛风的一种特殊关节外表现。其形成源于长期高尿酸血症导致单钠尿酸盐过饱和析出,这些坚硬结晶团块逐渐占据椎管空间,直接压迫脊髓或神经根,引发一系列神经系统症状。诊断需结合痛风病史、影像学检查及血尿酸水平综合判断。治疗关键在于及时手术减压切除结石,并结合长期降尿酸药物与生活方式干预,以控制病情并预防复发。
Abstract: Gouty stones in the spinal canal are a rare condition where urate crystals deposit within the spinal canal, representing a special extra-articular manifestation of gout. Its formation results from the long-term hyperuricemia leading to the supersaturation and precipitation of monosodium urate, with these hard crystalline masses gradually occupying the spinal canal space, directly compressing the spinal cord or nerve roots and causing a series of neurological symptoms. Diagnosis requires a comprehensive assessment combining a history of gout, imaging studies, and blood uric acid levels. The key to treatment lies in timely surgical decompression and removal of the tophi, along with long-term uric acid-lowering medications and lifestyle modifications to control the condition and prevent recurrence.
