摘要: 目的:探讨白介素6 (IL-6)和D-二聚体(D-D)与过敏性紫癜(HSP)预后的关系,为临床评估病情严重程度及预测复发或继发肾炎提供依据并可早期干预。方法:选取2023年5月至2025年5月安徽医科大学第四附属医院医院普儿病区收治且随访至2025年11月的4~12岁过敏性紫癜的儿童作为研究对象,并根据半年内是否出现紫癜复发或肾脏损害分为预后不良组(n = 24)和预后良好组(n = 25),比较两组患儿治疗前后IL-6及D-D数值,结果:1) 组间比较显示,治疗前预后不良组的患者D-D,IL-6浓度高于预后良好组;2) 与治疗前相比,预后不良组患者血清中D-D,IL-6浓度呈现出显著下降趋势;结论:IL-6和D-D是评估HSP预后的重要指标,联合检测可为临床干预提供参考,改善患儿预后。
Abstract: Objective: To investigate the association between interleukin-6 (IL-6) and D-dimer (D-D) with the prognosis of Henoch-Schonlein purpura (HSP), in order to provide a basis for clinical assessment of disease severity and prediction of recurrence or secondary nephritis, enabling early intervention. Methods: A total of 4~12-year-old children with HSP, admitted to the General Pediatrics Department of the Fourth Affiliated Hospital of Anhui Medical University between May 2023 and May 2025 and followed up until November 2025, were selected as the study subjects. Based on the occurrence of purpura recurrence or renal damage within six months, they were divided into a poor prognosis group (n = 24) and a good prognosis group (n = 25). The levels of IL-6 and D-D before and after treatment were compared between the two groups. Results: 1) The levels of D-D and IL-6 before treatment were significantly higher in the poor prognosis group compared to the good prognosis group. 2) In the poor prognosis group, the levels of D-D and IL-6 after treatment were significantly lower than those before treatment. Conclusion: IL-6 and D-D are important indicators for assessing the prognosis of HSP. Their combined detection can provide a reference for clinical intervention, thereby improving the prognosis of affected children.
