Treatment of Chronic Hand Eczema with Blood-Activating and Blood-Nourishing Methods

摘要: 慢性手部湿疹本质上是一种过敏性炎症性皮肤病，其发生与多种内外因素有关，临床表现以明显渗出倾向为特征。其临床表现多以皮肤皲裂、增厚，呈苔藓样变为主，急性期则伴有渗出等变化。大部分慢性湿疹目前找不到明确的病因，西医从内源性和外源性两方面因素认识它；中医认为正虚是发病的根本，湿邪为主要的致病因素，且易留恋不去，贯穿始终。实则外感风寒，内生湿热，久则化瘀，所以在治疗时应多以养血益气为主。临床上针对慢性湿疹的治疗，西医以口服西药和涂抹激素药物为主，中医根据辨证论治应用汤药、药物熏洗、针灸等手段进行治疗，均被证实具有较好的效果，而且若联合用药更能发挥其作用，使得临床症状得到明显改善。

Abstract: Chronic hand eczema is essentially an allergic inflammatory skin disease, which is related to various internal and external factors. Its clinical manifestations are characterized by obvious exudation tendency. Its clinical manifestations are mainly characterized by skin fissures, thickening and lichenification. In the acute stage, there are also changes such as exudation. Most chronic eczemas have no clear cause. Western medicine recognizes it from both internal and external factors. Traditional Chinese medicine believes that deficiency of the body’s vital energy is the fundamental cause of the disease, and dampness is the main pathogenic factor, which is difficult to eliminate and persists throughout the course of the disease. The actual cause is external exposure to wind and cold and internal generation of damp heat, which eventually leads to blood stasis. Therefore, in treatment, blood-nourishing and qi-tonifying methods should be the main approach. In clinical practice, the treatment of chronic eczema mainly involves oral administration of Western medicine and topical application of hormone drugs in Western medicine, while in traditional Chinese medicine, treatment is based on syndrome differentiation and involves the use of decoctions, herbal baths, acupuncture, etc., all of which have been proven to be effective. Moreover, combined use of drugs can enhance their effects and significantly improve clinical symptoms.
