Progress in the Diagnosis and Treatment of Arterio‑Ureteral Fistula
摘要: 动脉–输尿管瘘(Arterio‑ureteral fistula, AUF)是泌尿外科术后罕见但危及生命的并发症,尤其在盆腔手术、放疗或长期留置输尿管导管后易发生。临床表现为间歇性肉眼血尿(97%~100%),可突发大出血致低血容量性休克,诊断困难,易延误治疗。本文结合最新系统综述与多篇病例报告,系统阐述AUF的流行病学特征、危险因素、临床表现、影像学诊断及治疗策略,强调多学科协作与早期干预的重要性,以提升临床诊治水平。
Abstract: Arterio‑ureteral fistula (AUF) is a rare but life‑threatening complication following urological surgery, especially after pelvic surgery, radiotherapy, or long‑term indwelling ureteral catheterization. Clinical manifestations include intermittent gross hematuria (97%~100%), which may progress to massive hemorrhage and hypovolemic shock. Diagnosis is challenging and often delayed. This review integrates recent systematic reviews and multiple case reports to summarize the epidemiology, risk factors, clinical features, imaging diagnosis, and treatment strategies of AUF, emphasizing the importance of multidisciplinary collaboration and early intervention to improve clinical outcomes.
