Prognostic Impact of Patients with Gastric Cancer Lymph Node Stage
摘要: 胃癌是全球范围内严重危害人类健康的普遍性恶性肿瘤之一,居于癌症死亡率的前列。其中,淋巴结转移被视作胃癌预后的重要因素。胃癌淋巴结分期(N分期)基于转移淋巴结的数量及位置,与患者的生存率密切相关。随着分子生物学及免疫学的发展,胃癌淋巴结分期的精细化和个体化越来越受到关注。本综述以系统性文献回顾和研究为基础,探讨了胃癌淋巴结分期对患者预后的影响因素,包括淋巴结转移数量、淋巴结比例、淋巴血管侵犯、微转移以及伴随的免疫状态等因素。综上所述,淋巴结分期作为胃癌分期系统的一部分,其精细化和个体化的探讨对于提高胃癌患者的预后具有至关重要的意义,而深入理解其与生物标志物之间的关联,将为致力于延长胃癌患者生存期与改善生活质量的临床实践提供有价值的指导。
Abstract: Gastric cancer is one of the prevalent malignancies globally that poses a significant threat to human health, ranking high in cancer mortality rates. Lymph node metastasis is considered a crucial factor for the prognosis of gastric cancer and plays a key role in the staging of this disease. The lymph node staging (N staging) of gastric cancer, which is based on the number and location of metastatic lymph nodes, is closely associated with patient survival rates. In recent years, with the advancement of molecular biology and immunology, personalized treatment for patients has become a focal point, and the refinement and individualization of gastric cancer lymph node staging have gained increasing attention. This review is based on a systematic literature review and research, discussing the factors affecting the prognosis of patients with gastric cancer lymph node staging, including the number of lymph node metastases, lymph node ratio, lymphatic-vascular invasion, micrometastasis, and accompanying immune status, among others. Particularly, in the context of advancements in molecular targeted therapy and research on immune checkpoint inhibitors, precision medicine offers new perspectives and treatment strategies for lymph node staging. In summary, as part of the gastric cancer staging system, the refinement and individualization of lymph node staging are of crucial importance for improving the prognosis of gastric cancer patients. A deeper understanding of the relationship between lymph node staging and biomarkers will provide valuable guidance for clinical practices aimed at extending the survival and enhancing the quality of life for patients with gastric cancer.
