基于“一气周流，土枢四象”论治胃食管反流性咳嗽

Treating Gastroesophageal Reflux Cough Based on the Theory of “Circulating Qi and the Earth Pivot with Four Elements”

摘要: 胃食管反流性咳嗽是临床慢性咳嗽的常见类型之一，但西医易漏诊、误诊且治疗效果不佳。受清代医家黄元御“一气周流、土枢四象”理论的启发，联系临床探讨胃食管反流性咳嗽的论治方法，认为此病的病机为“阳虚土湿，气滞痰凝；内外受邪，胃逆肺寒”，据此提出“温中燥土，利气行郁；外散风寒，降逆温肺”的治法，为临床治疗胃食管反流性咳嗽提供了另一种诊疗思路。

Abstract: Gastroesophageal reflux cough is one of the common types of clinical chronic cough, but it is easily missed and misdiagnosed by Western medicine, and the treatment effect is poor. Inspired by the Qing Dynasty physician Huang Yuanyu’s theory of “circulating qi and the earth pivot with four elements”, we discussed the treatment methods of gastroesophageal reflux cough in connection with clinical practice, and believed that the pathogenesis of this disease is “yang deficiency and earth dampness, Qi stagnation and phlegm coagulation; internal and external combined pathogens, stomach-qi upstroke and lung cold”, based on this, the treatment method of “warming the dry soil in the middle, promoting Qi and promoting stagnation; expelling wind and cold externally, lowering inversion and warming the lungs” is proposed, which provides another way of clinical treatment for gastroesophageal reflux cough.
